This affidavit must be submitted for every LCUonline™ course you take.

Do not submit this form until you have completed all of the following requirements.

I have read all of the required reading. *
I have viewed and/or listened to all course lectures. *
I have turned in all required course research paper(s) (if any) by submitting a Research Paper Affidavit online. *

Attach Your Written Final Exam

You may submit your exam as:

  • A PDF file containing a scan of your exam;
  • Multiple JPG or PNG photos from your smartphone, or;
  • A ZIP archive containing either.

NOTE: All images MUST be well-lit, sharply focused, and easily readable. Any images failing these criteria will cause your exam to be rejected until usable images are submitted.
